Personalised Care Project Lead (Band 7)
An exciting opportunity has arisen in supporting the prevention and personalised care agenda in North Cumbria. Working in the CLIC Team, this fixed term post will help build the knowledge and skills in health prevention and personalised care of health and care staff and those living and working in communities who have the opportunity to hold health and well-being conversations.
The role will focus on planning, delivering and evaluating a personalised care project that:
- Builds awareness of personalised care and skills to hold a personalised conversation that helps people to know how to stay well and overcome the obstacles that prevent them from leading healthy lifestyles
- Ensures those health and care staff working in specific roles access the training that is available from the North East North Cumbria Integrated Care System
- Evaluates the impact of basic awareness training
- Communicates and promotes the importance of personalised care conversations and training available
