Shared decision making (SDM) is a process by which people are supported to understand their options and are given the opportunity to consider relevant information that might influence their choice. Shared decision making is part of the NHS Long Term Plan’s commitment to make personalised care business as usual across the health and care system.

New Teachback video for personalised care 

The Personalised Care Institute (PCI) has launched a short video on Teachback consultations, login required, which is a follow on from its Shared Decision Making (SDM) module. Teachback is an evidence based technique for clinicians to use during consultations to ensure people understand information they have been provided and can make use of it. It is particularly helpful when talking to people with limited or lower levels of health literacy, supporting them to participate in decisions and choices about their health and care. Personalised Care – Start the Conversation

The course will cover an introduction to what Personalised Care is and the skills needed to begin embedding Personalised Care conversations and Shared Decision Making in Health and Care. It will challenge practice with the aim to changing mind-set of workers and volunteers so that personalised care becomes part of everyday practice. This will hopefully enable staff to also share best practice and will therefore impact positively on service user and patient outcomes.

  • Embed a Personalised Care approach in practice
  • Enable staff and volunteers across Health and Care to start a Personalised Care Conversation

The role of Personalised Care in tackling health inequalities

The Covid-19 pandemic has exacerbated and has brought pre-existing health inequalities into sharp focus. But what can we learn from the last 2 years and how can health professionals close the health gap? What can healthcare professionals do when individuals facing greatest deprivation are not only experiencing higher risk of exposure to the virus whilst already living with existing poor health and multiple comorbidities?

Through a series of practical examples and case studies showcased by our guests, this episode explores the role of personalised care in reducing health inequalities through shared decision making, multidisciplinary collaboration, local community connections, patient engagement and patient empowerment.  

This episode is being hosted by Personalised Care Institute Ambassador Dr Bogdan Chiva Giurca who is joined by three very special guests: Dr Bola Owalabi, Director of Health Inequalities NHS England and NHS Improvement, Dr Selena Stellman – General Practitioner, and Dr Benjamin Ellis – Consultant Rheumatologist.

Connect 5 Mental Wellbeing Training

Connect5 is a mental health promotion training programme that increases the confidence and core skills of staff and volunteers so that they are more effective in having conversations about mental health and wellbeing in a Personalised way. Connect5 focuses on training individuals to enable them to support better mental health, resilience, and ultimately improved wellbeing.
