CLIC, in collaboration with North Cumbria Knowledge and Library Services, are planning a series of Lunch and Learns. These are 30 minute, virtual, informal sessions that take place at lunchtime - while you take a break and eat, you can listen to our invited speaker give a short talk sharing their experience of improvement work. The session is then open to anyone who has questions or reflections to offer.
Join us for this Lunch and Learn to hear from Clare Edwards (Health Partnerships Manager, Cumbria CVS), Sarah Farragher (Home First and System Flow Coordinator, Cumbria County Council, NENC ICB & North Cumbria Integrated Care NHS FT), and Sarah Thompson (Grants & Programmes Officer, Cumbria Community Foundation) on 'Effective partnership working – learning from the Well@Home project'.
This session will describe the fantastic partnership working which has taken place to establish the Well@Home service in north Cumbria.
From initial contact to having an up-and-running service, it took just six weeks of planning and organisation – a huge achievement for all who have been involved.
Well@Home is a real lifeline for individuals who are discharged from hospital without the help of family or friends, providing four weeks of support to enable them to regain and maintain their independence at home and in the community.
