Personalised Care – Start the Conversation (Cohort 11)

WHO IS IT FOR?

This is for all health and social care staff and volunteers including 3rd sector.

ABOUT THE SESSION

The course will cover an introduction to what Personalised Care is and the skills needed to begin embedding Personalised Care conversations and Shared Decision Making in Health and Care. It will challenge practice with the aim to changing mind-set of workers and volunteers so that personalised care becomes part of everyday practice. This will hopefully enable staff to also share best practice and will therefore impact positively on service user and patient outcomes.

AIMS

• Improve knowledge of Personalised Care
• Introduce the Model of Personalised Care
• Learn about how coaching  skills can improve delivery  of Personalised Care
• Learn about Support and Services available to signpost to
• Practice embedding Personalised Care conversations in your role
• Look at tools to aid with Personalised Care

OBJECTIVES

• Improve knowledge of Personalised Care.
• Embed a Personalised Care approach in practice.
• Enable staff and volunteers across Health and Care to start a Personalised Care Conversation.
• Encourage staff to share their approach and knowledge with others.
• Improve care delivery and outcomes for service users across the sectors by working in coproduction.

PRE-WORK

Attendees to access the Personalised Care Institute (PCI) Website and watch the video on Bringing Personalised Care to Life: What is personalised care? (personalisedcareinstitute.org.uk)  
There will also be a quick survey sent prior to training for completion.
