STOP AND WATCH Training (Cohort 28)

This training is for Staff Members of Care Homes and Support Agencies and family members of these service users

STOP AND WATCH is part of a National development to improve the early recognition of deteriorating health in residents of Care Homes and clients of Support Agencies. This workshop aims to help enable you to spot the early signs of deterioration and will introduce a useful reporting mechanism.

The STOP AND WATCH system gives a concise and easy to follow format that can be used by anybody to identify these early warning signs:

Seems different to usual

Talks or communicates less

Overall needs more help

Pain (new or worsening), participating in less activity

Ate less, lack of appetite

No bowel movement in 3 days or diarrhoea

Drank less; monitor using a hydration chart

Weight change; losing weight without trying

Agitated or more nervous than usual

Tired, weak, confused

Change in skin colour or condition

Help with walking, transferring or going to the toilet more than usual
